South Carolina targets measurable reductions in late-stage colorectal and lung cancer diagnoses among adults earning below 200 percent of the federal poverty level in the state’s 14 Appalachian counties and 10 coastal plain counties. Current surveillance data indicate that 37 percent of colorectal cancers in these counties are detected at stage III or IV, versus 29 percent in the remaining 22 counties.
The disparity traces to documented differences in primary-care visit frequency and absence of fixed endoscopy suites outside the Columbia and Charleston metropolitan statistical areas. Low-income households in these counties average 2.1 primary-care visits per year, compared with 3.4 visits statewide, limiting opportunities for risk assessment and referral.
The funding mechanism prioritizes projects that can report stage-shift metrics within 36 months. Successful applications have previously shown a minimum 8-percentage-point reduction in late-stage incidence by pairing community health centers with mobile colonoscopy units routed through the 14 Appalachian counties.
Reviewers examine whether proposed outcome measures align with South Carolina’s existing cancer registry fields, including exact county of residence at diagnosis. Proposals must also specify how findings will be stratified by Medicaid enrollment status, given that 29 percent of the target population relies on Medicaid for coverage.
Applications that cannot produce county-level baseline incidence tables from the South Carolina Central Cancer Registry within the first six months are typically returned without review. This requirement distinguishes South Carolina submissions from those submitted by states whose registries aggregate data only at the regional level.
