South Carolina maintains three academic medical centers but operates only one dedicated leukemia tissue repository, located in Charleston. The state’s 3.2 million rural residents, particularly in the Midlands and Upstate regions, receive care through community hospitals that lack standardized protocols for serial sample collection required by longitudinal studies. This operational fragmentation limits the ability of early-stage investigators to assemble cohorts large enough to examine treatment outcomes across payer types.

Collaborative treatment programs must therefore begin with formal data-use agreements among the Medical University of South Carolina, Prisma Health, and the South Carolina Cancer Alliance. These agreements enable sharing of electronic pathology reports while complying with state privacy statutes that differ from neighboring North Carolina. Without such coordination, duplicate enrollment and loss to follow-up rates exceed 30 percent within the first year of observation.

Grant funds can support a project manager position to maintain a centralized REDCap database hosted by the state health department, covering personnel and limited travel for quarterly site visits to Greenville and Columbia. Supplies budgets accommodate standardized collection kits shipped to participating community oncology practices, ensuring uniform processing of peripheral blood and marrow aspirates.
