South Carolina maintains a documented shortfall of 1,800 community health workers relative to population needs, with the largest deficits concentrated in the 13 counties designated as Health Professional Shortage Areas by the federal government. This deficit is measured against a target ratio of one worker per 1,500 residents in rural districts.
The existing workforce is concentrated in the Upstate manufacturing corridors and Charleston metro, leaving the coastal plain and Pee Dee regions with minimal coverage. African American residents, who form 27 percent of the state population, experience higher rates of chronic disease yet lower contact with outreach personnel. Agricultural and tourism employment patterns create irregular schedules that further limit participation in centralized training programs based in Columbia.
Funding supports short-term placements of medical trainees who can deliver structured modules on chronic disease screening, medication adherence, and local resource navigation to incumbent community health workers. Sessions are scheduled around harvest cycles in the 10 highest-need counties and incorporate state-specific vital statistics on diabetes and hypertension.
Applicants must supply letters from South Carolina Department of Health and Environmental Control district offices verifying that training sites meet HIPAA and state licensing thresholds. Proposals are evaluated on whether at least 70 percent of trainee time is allocated to direct instruction rather than observation. Priority is given to placements that produce training materials in both English and Gullah/Geechee dialects used in lowcountry communities.
Applications originating from neighboring states are distinguished by the requirement to show explicit coordination with South Carolina’s existing community health worker certification registry, a step not uniformly applied in Georgia or North Carolina submissions.
